Home support from social worker and nurse for addictions
* integrated assessment and treatment plan to assist people in managing their withdrawal symptoms from alcohol/drugs

* provides family or support system with the knowledge and support to minimize the risks during withdrawal
* daily monitoring of withdrawal symptoms
* home visits/in office supportive counselling
* 24 hr telephone support
* facilitate tampering plan
* facilitates family physician involvement
* facilitates access to appropriate levels of care i.e. emergency care or medical withdrawal management services if necessary
* reduces barriers to obtaining withdrawal management services
* reduces risk of withdrawal complications
* provides clients with several options of treatment services after they have completed their withdrawal experience
An "intake" involves asking some questions like name, phone number, address and some details about the situation in order to initiate the assessment process * intakes are usually completed by telephone after attending an information session
