Glossary

Name The official operating name of the organization or program, usually as it appears on letterhead. If the organization or program is commonly known by another name (an acronym, for example), make a note in the comment box.

Office phone

Main telephone number. If there is more than one number, please specify how each number should be used. For example: 416-123-1234 Administration, 416-234-2345 Intake. Extension numbers can be used.

Crisis phone

Telephone number to call in a crisis or emergency. This number can be the same as Office phone. Service must be available Monday-Sunday 24 hours.

TTY phone

Teletype machine or telecommunication device for the deaf. If there is more than one number, please specify how each number should be used. For example: 416-123-1234 Administration, 416-234-2345 Intake.

Toll-free phone

Telephone number that allows callers to call free of charge. If this number should only be used for a particular service, please specify. For example: 1-800-123-1234 Information Hotline.

Fax

Fax machine number. If there is more than one number, please specify how each number should be used. For example: 416-123-1234 Administration, 416-234-2345 Emergency Department.

Email

Main email address for the organization or program.

Web site

Main home page for the organization or program on the World Wide Web.

Address

Street address where the organization or program is physically located. If this location is confidential or not open to the public, complete the Mail Address only! Make a note in the comment box that the service has no street address.

Mail address

Address where mail should be sent if different from street address or if street location is confidential or not open to the public.
Executive Officer 1 Name and title of the most senior person in the organization. This person is usually a staff person.

Executive Officer 2

Name and title of a senior person in the organization who shares responsibility with Officer 1 or can be contacted if Officer 1 is unavailable. This person is usually a staff person.
Contact Person 1 Name and title of the person to contact for information or service, if different from Officers 1 or 2.

Contact Person 2

Name and title of a person to contact for information or service if Contact 1 is unavailable.

Hours

Days and hours of service including opening and closing times of main organization. Hours also includes a breakdown of days and hours of service for programs, where applicable.

Area served

Geographical boundaries of service. This can be all of Canada, Ontario, a region or county, or the City of Toronto. It can also be a part of Ontario that includes the City of Toronto or an area within the City of Toronto. Boundaries are indicated in the order of North-South, East-West. For example: Steeles Ave-Lake Ontario; Bayview Ave-Etobicoke.

Languages

Languages that service is consistently provided in. English is assumed to be the default language of service. If service is not available in English, make a note in the comments box. Languages are listed in alphabetical order. Spelling is assigned by Findhelp Information Services.

Fees

Fees for service, if any. Information about subsidies is also included. Does not include fees for general membership unless membership is required to receive services.

Eligibility

Specific criteria that people must meet to receive service. Some common criteria are area of residence, gender, age, income level, receipt of benefits, type of disability and ethnocultural community.

How to apply

First step that people should take to receive service such as calling for an appointment, dropping by or completing an application form.

Physical access

Accessibility for people with physical disabilities including details about entrances, washrooms, elevators and parking. It can also include details about services located on specific floors of a building. Findhelp Information Services assigns standard wording. Helpful phrases include "Wheelchair accessible building including main entrance and barrier free washrooms," "Braille and tone elevator," "Designated parking" and "Street parking close to entrance."
